In my latest story for Health Affairs, I profile a model of care at a public hospital in Memphis that aims to address the challenges of treating its uninsured population. The hospital, with a team of social workers and nurses, connects uninsured patients with local social services organizations and coordinates their care so they can get help with chronic illnesses outside the hospital. The program is helping people live healthier lives and is saving the hospital money. See my story here.
